By scheduling an appointment, you are acknowledging that you have read and agreed to the IMPORTANT INFORMATION, MICROPIGMENTATION CONTRADICTIONS, PRE-TREATMENT INSTRUCTIONS and THE FINE PRINT.

•PRE-TREATMENT INSTRUCTIONS•

YOU MUST AVOID THE FOLLOWING BEFORE YOUR APPOINTMENT

6 MONTHS PRIOR TO YOUR APPOINTMENT

  • You must be off of Accutane for at least 6 months prior to any procedure.

8 WEEKS PRIOR TO YOUR APPOINTMENT

  • No eyelash or eyebrow growth serum (Latisse, etc) for eyeliner and eyebrows.

4 WEEKS PRIOR TO YOUR APPOINTMENT

  • Eyelash extensions must be removed at least 4 weeks prior to your appointment for eyeliner.

  • No chemical peels, microdermabrasion, facials or laser skin treatment for 4 weeks prior to your procedure.

2 WEEKS PRIOR TO YOUR APPOINTMENT

  • No Botox or Dermal Fillers 2 weeks prior to your procedure.

  • Discontinue use of topical Retinol, Alpha-Hydroxy Acid, Glycolic Acid, Salicylic Acid, Tretinoin on or around the treatment area for two weeks prior to your procedure.

  • The above listed ingredients will cause pigment to fade prematurely.

  • Consider avoiding the use of these on the treated area, indefinitely.

  • .Avoid direct sun and tanning beds for 14 days prior to your procedure and 14 days after your procedure.

  • Do not have a tan/sunburn on your face, scalp, or body prior to your procedure.

    Arriving to your appointment having had sun exposure will result in having to reschedule.

  • Avoid tinting, dyeing, waxing, plucking or threading your eyebrows.

  • You must wait two weeks after receiving a vaccination or booster shot to have a procedure.

  • You must have completed any antibiotic or steroid treatment at least two weeks prior to your procedure.

1 WEEK PRIOR TO YOUR APPOINTMENT

  • Avoid vitamins such as vitamin a, vitamin e, b6, Niacin, Vitamin C, Flaxseed Oil, Fish Oil, Omega 3, Turmeric, Biotin, Prenatal Vitamins, power shakes, and green shakes. Limit consumption of seafood high in Omegas.

  • Do not take blood thinners (Aspirin, Heparin, Warfarin, Coumadin), or NSAIDS (Ibuprofen, Aleve) for 3 days prior to your procedure.

    The blood thinners listed above will cause excessive bleeding and swelling. Bleeding prevents pigment from being implanted effectively. It will also prevent you from retaining pigment, by causing unnecessary scabbing and very unsatisfactory results.

    24 HOURS BEFORE YOUR APPOINTMENT

  • Avoid alcohol use the night before your procedure

THE MORNING OF YOUR PROCEDURE

  • Avoid excessive sweating and any high impact exercise.

  • AVOID ALL FORMS OF CAFFEINE

If you arrive to your appointment, having consumed caffeine, unfortunately your appointment will have to be rescheduled.

•MICROPIGMENTATION CONTRADICTIONS•

YOU ARE NOT THE IDEAL CANDIDATE IF YOU HAVE ANY OF THE FOLLOWING,,,

  • Under 18 years of age

  • Pregnant or breastfeeding

  • Have received a vaccine or booster shot within the last three weeks 

  • Heart Conditions: Pacemaker/ Defibrillator/Mitral Valve Prolapse

  • Uncontrolled Low/High Blood Pressure

  • Allergies to nickel, iron oxide, anesthetics, hair dye or makeup

  • Uncontrolled Diabetes

  • Liver Disease

  • Trichotillomania

  • Epilepsy/Seizure Disorder

  • Those undergoing active cancer treatment (Chemotherapy, Radiation)

  • Have taken Accutane or steroids in the last 6 months

  • Are prone to post-inflammatory hyperpigmentation

  • Skin conditions such as eczema, psoriasis, rash, or blisters on or around the procedure area 

  • Active Herpes outbreak

  • Prone to keloid scars

  • Botox or  dermal fillers in the last three weeks

  • Active viral infections, COVID-19, a cold, or any flu-like virus

  • Anyone who has undergone an organ transplant

  • Active skin cancer on or around the procedure area 

  • Healing disorders

  • Anemia

  • Hepatitis, HIV, Hemophilia, or any platelet or bleeding disorder

  • Drug/Alcohol abuse/Nicotine use

  • Anyone taking blood thinners as a medical necessity

  • Use of eyebrow/eyelash growth serum in the last 8 weeks (for eyeliner and eyebrow procedures)

  • Glaucoma or use of glaucoma drops in the eyes (eyeliner procedures)

  • Eyelash extensions in the last 4 weeks (eyeliner procedures)

  • Blepharoplasty within the last 12 weeks (eyeliner procedures)

Note: This does not always mean that you can not have a procedure done. For some of the above conditions, there are exceptions, although a physician's release will be required.

IMPORTANT INFORMATION ABOUT EYEBROW PROCEDURES

  • All Micropigmentation is achieved in multiple sessions.

  • All eyebrow procedures require a minimum of two sessions to complete.

  • 6-8 weeks following the initial session, is a Touch-Up appointment. Most people only need these two sessions although some individuals will require an additional Touch-Up.

  • Touch-Up appointments are reserved only for existing clients who have had their eyebrows done by me within the last 12 weeks.

  • Maintenance appointments are annual or every two years. These are also reserved for my existing clients wanting a color boost to their brows after they have faded about 50%. The timing of these appointments vary from person to person depending on lifestyle, skin type and sun exposure.

    • It is advised that you book a Phone Consultation.

    • It is NOT considered a touch-up appointment if the work was not done by A DOSE OF INK.

    • After submitting a photo, please wait for my response, as to whether I can work on you. I will let you know whether you should book a Color Correction/Coverup or a New Appointment.

    WHAT IS THE COST?

    Touch Up (6-8 weeks) • $150.00

    Additional Touch Up (12-16 weeks)  • $75.00

    6-12 Month Color Refresh • $350.00

    1-2 Year Annual Color Refresh • $450.00

    2+ Year Color Refresh • $550.00

THE FINE PRINT

BOOKING FEES

  • A $50 non-refundable booking fee is required to secure your appointment. This will be applied toward the balance of your procedure. This booking fee is non-refundable.

  • Booking fees are transferrable one-time only.

  • I understand unforeseen circumstances may arise that could force you to cancel/reschedule your procedure. Please understand that such changes affect not only your technician, but other clients as well. If you choose to reschedule your appointment, a minimum 48-hour advance notice is required, or the booking fee becomes non-refundable and non-transferable and an additional deposit will be required to make any future appointments.

    CANCELLATIONS/ RESCHEDULING/ LATE ARRIVALS/ NO-SHOWS

  • Please arrive at your scheduled appointment time. Please do not arrive too early, as I may still be with another client or disinfecting and sanitizing the studio in preparation for your appointment.

  • To avoid unfair delays to other clients, please arrive at your scheduled appointment time. I do understand unforeseen circumstances may arise, If you are more than 15 minutes late to your appointment, please contact me immediately, as I will try my best to proceed with your appointment. If time will not allow, we may need to reschedule, If we do need to reschedule, your booking fee will be applied to the new appointment.

  • If you do not contact me, your appointment will be cancelled, and you will forfeit your booking fee. If you decide to reschedule for a later date, you will be asked to repay a $50 booking fee.

  • Any client that does NOT show for their scheduled appointment, will forfeit their $50 booking fee and will be required to pay for any future appointments in full prior to their appointment.

  • If you have any signs or symptoms of Cold, Flu or Covid 19 (fever, cough, shortness of breath, headache, body aches) please notify me immediately to reschedule your appointment. You WILL NOT be charged a cancellation fee and you can transfer your deposit for a future appointment.

PAYMENTS

  • Your $50 booking fee will be applied to the balance of your treatment.

  • The remainder of your payment is due on the day of your appointment, unless a payment plan or other arrangements have been made prior to your scheduled appointment.

  • A Dose Of Ink accepts Credit Cards (Visa, Mastercard, American Express, Discover), Zelle, Venmo, Cash and Gift Cards.

DISCLAIMER

  • Results may vary depending on compliance of aftercare instructions, skin type, and your lifestyle. Your treatment is our responsibility while you are at our studio. Following your procedure, preservation and longevity of your service is your responsibility. Please continue to follow proper aftercare instructions for the best results moving forward.

  • Quoted price is subject to change if additional services are provided during your appointment.

  • Photos and videos may be taken during the appointment for marketing purposes.

    If you are not comfortable with this, and you have declined this on your consent forms, please also confirm your decision verbally with me at your appointment.

  • Due to insurance regulations, NO additional guests are allowed in the treatment room during your procedure. Although they are certainly welcome to sit in our waiting area.
